Macau Dental Clinic - Periodontal Treatment Guide

What is periodontal disease?

Periodontal disease refers to the disease of the tissues around the teeth, including dontitis and periodontitis. The most common cause is chronic inflammation of the periodontal tissues caused by inadequate oral care. Smoking and poorly controlled diabetes both increase the risk of periodontal disease. Studies have shown that periodontal disease can increase the risk of heart disease, so it should not be taken lightly. The initial signs of periodontal disease include redness and bleeding of the teeth. Some people, especially smokers, may not have symptoms at all. In serious cases, the teeth will shrink, have bad breath, weak biting, and even easily shake the teeth. Therefore, it is necessary to do oral examination regularly and deal with the problem in the early stage of periodontal disease. If periodontal disease is severe, the loose tooth may have to be removed. According to 2011 government statistics, 98.6% of adults aged 35 to 44 suffer from tooth bleeding, while 39.6% are affected by periodontitis. It can be seen from this that periodontal disease is a relatively common disease in Hong Kong, and if patients seek medical treatment at the early stages of periodontal disease, the chance of tooth loss due to periodontal disease will be reduced in the future.

The cause of periodontal disease is mainly caused by bacteria. There are many bacteria in the human mouth, some of which can cause periodontal disease. If bacteria stay on your teeth for a long time, plaque forms. When plaque calcifies, it turns into tartar. Calculus will make it easier for bacteria to grow on the surface, which will inflame nearby teeth. When the teeth become inflamed, swelling and bleeding occur. If the tooth meat is inflamed for a long time, the tooth meat and bone will gradually atrophy, the tooth will be exposed, and eventually cause the tooth to loosen and shake the tooth. Therefore, if we can address the causes of periodontal disease, we can greatly reduce the risk of developing periodontal disease. Regular dental cleaning to remove calculus can reduce the accumulation of bacteria and reduce the chance of developing periodontal disease. Apart from the causes of periodontal disease mentioned above, there are a number of risk factors that increase the chance of developing periodontal disease. Common risk factors include smoking, diabetes, stress, genetic factors, dental disalignment, poor oral care, hormonal factors, and medication.

There are many signs of periodontal disease, and each patient's condition may be different. The most common signs include: bad breath, swollen teeth, sore teeth, bleeding teeth, weak biting, loose teeth, shrunken teeth, sensitive teeth, occlusal changes.

Degree classification of periodontal disease

In 2017, the periodontal academic community reclassified the severity of periodontal disease, roughly classifying the current situation of periodontal disease and the course of disease in the past. The current situation of periodontal disease can be divided into the first stage to the fourth stage, the first stage is the milder, the fourth stage is the most serious. Indicators include the depth of periodontal tissue loss, the bone loss seen on the X-ray, and the location of fractures affecting the main teeth. The progression of periodontal disease can be divided into grades A to C, with Grade A progressing the slowest and Grade C progressing the fastest. The progression of periodontal disease is influenced by many factors, including the patient's lack of smoking, diabetes index and oral hygiene. If you suspect that you have periodontal disease, you should go to the dentist's office for an oral examination as soon as possible. Then the dentist can classify your current periodontal disease condition and its progress, and provide appropriate treatment options to deal with the causes of periodontal disease.

The treatment of periodontal disease mainly includes several steps:

Full oral examination: The doctor will conduct a full oral examination first, including tooth decay examination and preliminary periodontal examination, to ensure that other oral problems are not overlooked. The doctor will then take a picture of your mouth for a record.

X-ray: For patients with moderate to severe periodontal disease, we will take an X-ray for each patient to assist in the diagnosis and management of periodontal disease.

Comprehensive periodontal examination: If your initial periodontal examination shows that your tooth has a periodontal pocket more than 3.5 mm deep, your doctor will perform a comprehensive periodontal chart for you to record the depth of the periodontal pocket, the degree of tooth shrinkage and the closeness of your tooth.

Periodontal treatment: Based on your comprehensive periodontal history, we will provide you with appropriate periodontal treatment, such as non-surgical deep cleaning. Each deep cleaning is usually done in two left and right sections to ensure that it does not affect your diet too much.

Home Oral Care Guidelines: After completing periodontal treatment, we will provide you with guidelines to improve your oral care, so that you can keep your teeth clean and your periodontal disease under control.

Follow-up visit: Three months after the completion of periodontal treatment, you should return for a follow-up visit to ensure that the periodontal treatment results are satisfactory.
